Imagine that non autonomous patients with chronicle diseases can cure themselves and receive assistance at home allowing for health workers and cooperatives to be informed and involved on the status of patients.

BUSINESS NEED
In a perspective of spending review for local health companies, the aim is to reduce hospitalizations (each patient has an average cost ranging between 700 and 1000 euros per day) and promote a solution that favors home assistance based on the patient’s problems.

SOLUTION
The system optimizes the association between health workers and treatments and defines an “activity plan” sent to the operators via smartphone so they can directly access their agenda of appointments. The system relies on two information systems that support the service delivery process managed by the cooperatives: a web portal and a mobile application.

RESULT
Social Care aims at improving the quality of life of non self-sufficient citizens affected by chronicle diseases along with their families converging two areas that today are still quite distant one from the other: home assistance and telemedicine.
